Guest Waiver

GUEST AGREEMENT AND RELEASE OF LIABILITY

This Guest Waiver applies to all guests attending a private party or event at Party à la Kart, 44799 Saranac St., Ashburn, VA. By submitting this form, the parent or legal guardian of the guest child ("Guest") acknowledges the following:

1. Assumption of Risk. Guest acknowledges that all recreational activities at Party à la Kart carry inherent risks. Guest voluntarily assumes all such risks on behalf of the child listed below.

2. Release of Liability. Guest hereby releases and discharges Party à la Kart, its owners, employees, agents, and affiliates from any and all claims arising out of participation in activities at the facility.

3. Medical Authorization. Guest authorizes Party à la Kart staff to obtain emergency medical treatment for the guest child if necessary and if the parent or guardian cannot be reached.

4. Compliance with Rules. Guest agrees that the guest child will comply with all facility rules and staff instructions throughout the event.

By completing the form below, Guest confirms they have read and agree to all terms of this guest waiver.

By signing below, the participant confirms their responsibility for their own safety and the safety of their child(ren) during the event. The participant also releases Party à la Kart and Karter Schools from any liability for injuries or accidents that may occur. Additionally, this document serves as a visitor log and will be shared with Karter Schools.

This Agreement is made and entered into by and between PARTY A LA KART, LLC, a Virginia limited liability company (“Party”), and (“Participant”). Party provides a venue and activities for special events for children, such as birthday and other parties, and Participant desires to attend or have their child(ren) participate in such events. Participant acknowledges that activities involving children may carry inherent risks of injury or damage. To ensure clarity and understanding, the parties agree as follows:



Event and Services Provided


Party will provide a venue and an entertainment environment for children to enjoy the event.



Assumption of Risk


Participant understands and acknowledges that there are inherent risks associated with children’s activities, including but not limited to physical injury, and voluntarily assumes all risks associated with participation.



Release and Indemnity


Participant does hereby release, indemnify, and hold harmless Party, its owners, members, and employees, from any claims, damages, or liabilities, including injury or harm, whether caused by negligent or intentional acts during the event.



Supervision Acknowledgement


Participant acknowledges that Party provides a safe and secure environment, including safety measures to prevent children from wandering or unauthorized access to the facility. However, Participant understands that they remain responsible for supervising their child(ren) during the event.



Cameras and Monitoring


Participant acknowledges that the venue is a childcare center equipped with cameras through Watch Me Grow, and activities may be monitored for safety.



Acknowledgment of Responsibility


By signing below, Participant acknowledges that they are responsible for their own safety and that of their child(ren) during the event. Participant releases Party a la Kart and Karter Schools from any liability for injuries or accidents that may occur.
